Child Dedication Application Form

If you wish to have your child(ren) dedicated at MLFC, please fill in the information requested below. The deadline to submit this form will be communicated during our church service. Thank You!

If you have already attended a Pre-Dedication Class, you do not need to attend the next one. However, in order to participate in the upcoming dedication ceremony, you still need to sign up your child. If you have any questions or concerns, please do not hesitate to contact us.

Child Dedication certificates can be collected from the Child Dedication Check-In table after the service.
